Shiftwave and roXiva RX1 are wellness experiences only. They are not medical, psychological, or psychiatric treatment or advice, and are not a substitute for professional medical care.
Do not participate if any of the following apply:
Pregnancy
Epilepsy, seizure history, or family history of epilepsy or seizures
Retinal tear or retinal detachment
Blood clots, thrombosis, or unstable angina
Recent use of alcohol, recreational drugs, or other psychoactive substances within the last 24 hours
Recent laser treatment to the eyes or face
Under 16 years of age without a parent or guardian present to sign this waiver on your behalf
Please disclose to staff before participating if any of the following apply:
Any heart condition
Recent surgery or surgical implant
Head or brain injury, brain surgery, or stroke
Prescribed medication use, especially photosensitive medication
Psychological or psychiatric conditions
Any condition that may be worsened by vibration, flickering light, or immersive audiovisual stimulation
Any concern about your tolerance to the above

I understand these are wellness experiences only and not medical, psychological, or psychiatric treatment or advice
I understand that flickering light, immersive audiovisual stimulation, and full-body vibration may cause discomfort and, in susceptible individuals, may trigger adverse reactions
I will immediately tell staff if I feel pain, dizziness, nausea, distress, or any unusual symptom, and I understand the session may be modified or stopped at any time
To the extent permitted by law, I accept the risks of participation and release Cheatcode Playground and its staff, contractors, and related entities from claims or liabilities arising from my participation, except where liability cannot legally be excluded

Cancellations must be made at least 24 hours in advance. No-shows are charged the full session price. A 10-minute grace period applies before a no-show fee is applied.
